What is osseointegration and why is it important?
Osseointegration is the process by which a titanium implant integrates with the jawbone. After placement, bone cells grow around the implant surface, permanently anchoring it to the bone — similar to a natural tooth root. This process typically takes 2–3 months in the lower jaw and 4–6 months in the upper jaw. Proper osseointegration is the foundation of implant success, determining long-term stability and durability.
Absolute contraindications include recent radiotherapy in the jaw area, treatment with bisphosphonates (osteoporosis medications), severe immune system disorders, pregnancy, and breastfeeding. Relative contraindications require individual assessment, such as controlled diabetes or smoking. Insufficient bone volume is not a contraindication — it can be rebuilt through augmentation procedures.
Bone augmentation is a procedure that rebuilds bone in areas where its volume is insufficient for implant placement. Bone loss occurs after tooth extraction (due to lack of mechanical stimulation) or as a result of periodontal disease. During augmentation, bone graft material (autogenous, synthetic, or xenogeneic) is placed in the deficient area. After 4–6 months, the material transforms into new bone suitable for implant placement. In the upper jaw, a sinus lift procedure — elevation of the sinus floor — may also be required.
Implant placement is performed under local anesthesia, so the patient does not feel pain during the procedure. After the anesthesia wears off, mild discomfort and swelling may occur for 1–3 days and are well controlled with standard pain medication. Most patients report that the discomfort is less than they expected. Swelling typically peaks on the second day and gradually subsides. After about a week, most patients feel no discomfort.
An implant requires the same hygiene as a natural tooth: brushing twice daily and cleaning interdental spaces every day — the use of a water flosser is especially recommended. Regular check-ups every 6 months, including professional hygiene visits, are essential. Smoking should be avoided (as it increases the risk of peri-implantitis — inflammation of tissues around the implant). Maintaining overall health, including proper diabetes control, and wearing a night guard in cases of teeth grinding are also important. Good oral hygiene is the key to long-term implant success.
